What we do:
Coordination of Care
Effective inter-professional collaboration to help prevent medication and other errors, including identified exacerbation of comorbidities
Health Literacy
Empower clients through education to promote adherence to medical instructions and self-management, record maintenance, primary-care and specialist follow up, and ensuring patients are knowledgeable about their conditions.
Polypharmacy
Collaborate with all client health providers to ensure appropriate care planning, the integration of medical, social, cultural, linguistic, and behavioral aspects of care with the ultimate goal of aiding client and family engagement, and health management.
Concierge Nursing
Addresses a wide variety of needs from pre/post-surgery care and medication administration to routine vital sign monitoring and catheter care.
Chronic Disease Management
According to the National Council of Aging “80% of older adults live with least one chronic disease, and 68% have at least two. Many adults with chronic conditions struggle to manage them.”
Disease education combined with nutrition and exercise provides confidence, knowledge of ways to manage the disease and motivation to better manage the challenges.
We focus on Diabetes, High blood pressure or Hypertension, Congestive Heart Failure, Urinary Tract Infections and many more.
