Poly pharmacy Management (Medication management)
We help with medication reconciliation and can provide weekly or monthly medication pill box set up, coordination with pharmacists for medication review, and ensure timely refills.
This is included in a limited nursing assessment and family report to ensure loved ones are safe at home. We can also administer subcutaneous or intramuscular provider-ordered injections like med administration-Lovenox injections, Vitamin B12 injections, Ozempic injections

For all of our clients, our work begins with serving as the patient’s liaison and advocate by collaborating with discharge planners, registered nurses, physicians, PT/OT/NP/PA/SLP, Respiratory therapists and home health care companies. This includes assisting with the setup and management of online health records to ensure ease in scheduling appointments and monitoring test results.
Care coordination is also essential to address care gaps and avoid service duplication as well as focusing on the care itself and providing resources such as community services and partnerships.

When facing chronic health issues, planning is key to the maintenance of health. Agility Heath is skilled at helping to establish care plans, in conjunction with physicians and families, that are easy to understand and follow.
These plans include knowing when and to whom to communicate changes in the client`s condition, identifying the need for treatment, and what is necessary for ongoing care. Through education and routine communication with clients, we are also able to provide
symptom management where we -discuss all symptoms and help escalate to providers, as needed.
